How an empty North Carolina rural hospital explains a GOP senator's vote against Trump's tax bill - ABC News

States that have declined to expand Medicaid coverage, the health insurance program for the poorest of Americans, have seen the closures accelerate. Tennessee, for example, has shed 500 beds since 2014, when a federal law first allowed states to expand Medicaid coverage to a greater share of low-income people. It’s one of 10 states that has not expanded Medicaid. More than 300 hospitals could be at risk for closure if the Republicans' bill becomes law, an analysis by the Cecil G. Sheps Center at the University of North Carolina at Chapel Hill found last month. The center tracks rural hospital closures. “Substantial cuts to Medicaid or Medicare payments could increase the number of unprofitable rural hospitals and elevate their risk of financial distress,” the analysis concluded. “In response, hospitals may be forced to reduce service lines, convert to a different type of health care facility, or close altogether.”An empty hospital in Williamston, North Carolina, offers an evocative illustration of why Republican Sen. Thom Tillis would buck his party and its leaders to vote down President Trump’s signature domestic policy packageMartin General is one of a dozen hospitals that have closed in North Carolina over the last two decades. This is a problem that hospital systems and health experts warn may only worsen if the legislation passes with its $1 trillion cuts to the Medicaid program and new restrictions on enrollment in the coverage.

Community health centers like Affinia Healthcare could lose critical funding from Medicaid if the “Big Beautiful Bill” passes. Even without changes to Medicaid, Affinia is already seeing cuts in the federal funding that helps it continue operating. Holmes said the health center recently had some of its immigrant and refugee funding cut, as well as grants for providing vaccines.Affinia Healthcare and other community health centers are Medicaid recipients.Community health centers serve all patients regardless of their ability to pay, meaning they rely heavily on funding sources other than private insurance.Holmes said the health center continues to rely heavily on grants, and does accept patients with private health insurance alongside Medicaid recipients and uninsured patients.

Health Center - an overview | ScienceDirect Topics

CHCs are 4 times as likely to see self-paying patients than a private physician's office. CHCs fall under a broad category of clinics that are designated as Federally Qualified Health Centers (FQHCs), a category that also includes some American Indian health clinics. Congress passed the Migrant Health Act of 1962 (P.L. 87-692), which provided for development of dedicated health clinics to serve the needs of these workers and families. President Lyndon Johnson's “War on Poverty” in the mid-1960s was responsible for passage of the Economic Opportunity Act of 1964, which added to community health centers by creating neighborhood health centers in urban areas with high poverty populations.Over the next two decades, new programs were added to the CHC program to include the Migrant Health Center program, Healthcare for the Homeless Centers, and the Public Housing Primary Care Program.27 Later, the Omnibus Budget Reconciliation Acts of 1989, 1990, and 1993 created a new entity under Medicare and Medicaid known as the Federally Qualified Health Center (FQHC); the Social Security Act also expanded the definitions to add amendments describing FQHC Look-Alikes.The Bush administration established the President's Health Centers Initiative in 2002, which will eventually increase the system by 1200 expanded or new access points, thereby pushing the number of patients served from approximately 15 million to 19.8 million.28 It is important to recognize that although the rural population is only 20% of the nation's total, slightly more than 32% of CHCs are located in rural areas with higher poverty, unemployment, less insurance coverage, and a growing minority population.29Even in the absence of IT, an AHC—when viewed as a system—must struggle with how the triumvirate of care, education, and research reinforce each other toward the common goal of human health. These primary components are further divided into medical facilities, academic departments, centers and institutes, and administrative and support units.
